Diak’s forms are fillable PDF forms or online Visma sign forms. When using our forms, please proceed as follows.

Pdf forms

1. Save the form to your computer’s hard drive, do not open it directly from the browser. This is important to ensure that all fields are filled and displayed correctly.
2. Open the form file in Adobe Reader.
3. Fill in the form.
4. Rename and save the form to your computer before uploading it to e.g. Diakle. This way teachers can see the field contents.

If a field doesn’t have enough space for your text, you should print the form and complete it by hand.

Visma sign online form

• The form is signed with Suomi.fi authentication. For that, you need a mobile certificate or online banking IDs.
• If you do not have them, contact Diak’s Study Office (opintotoimisto@diak.fi).

Use this form only if you don’t have the possibility to register as attending or non-attending in MyDiak. Further information on registration.

The form is signed with Suomi.fi authentication. For that, you need a mobile certificate or online banking IDs. If you do not have them, contact Diak’s Study Office (opintotoimisto@diak.fi).

Use this form if you wish to resign from your studies at Diak. We strongly recommend that you discuss your situation with your study guidance counsellor before filling this form and resigning from your studies. Instead of resigning there may be alternative solutions suitable for your situation, which you can discuss with your study guidance counsellor. We would also appreciate the opportunity to discuss your reasons for withdrawing so that we can improve our operations. 

The notice of resignation is an electronic Visma Sign form, which must be signed using strong authentication (e.g. Finnish online banking credentials or a mobile certificate). If you are unable to use strong authentication, please contact the student services office to obtain a paper form.

Visma sign form. In case you can’t fill the form due to the lack of online bank service ID, mobile certificate or personal ID number, please contact: opintotoimisto@diak.fi.

Fill Agreement on the copyright form as an attachment of Thesis agreement form, if your thesis produces an outcome, for example a guide, a video, etc.

Use this form to describe the processing of your thesis data and attach it as part of your thesis plan. Note: The form does not apply to descriptive literature reviews.

If your thesis involves the processing of personal data, prepare a privacy statement to inform participants about the processing of personal data. You can use Diak’s privacy statement form to prepare it. Fill out the form. You will receive a link to the completed privacy statement in your email. Download and save the statement for yourself immediately.

The form is only filled out if sensitive personal data is processed. As a rule, students should not create theses that include the collection of sensitive personal data, in which case a data protection impact assessment (DPIA) is not required.

Use the thesis consent form when you need permission from an interviewee or a participant of your development project.

Fill out the Research permit application and its attachments when you are conducting research on Diak as an organisation, or on Diak students or staff. By clicking ‘Research permit application’, you will receive further instructions. When you are applying for a research permit from another organisation and they do not have the necessary forms, you can use Diak’s forms as a model.
