On this page, we have collected all of the PDF forms for students. In addition, there are also forms for placement supervisors.

Please note, for instance, that the primary registration channel for the academic year is MyDiak. Use the PDF forms only in case you are not able to log in on MyDiak.

Instructions for filling and printing forms

Diak’s forms are fillable PDF forms, unless otherwise specified. When using our forms, please proceed as follows:

  1. Save the form to your computer’s hard drive, do not open it directly from the browser. This is important to ensure that all fields are filled and displayed correctly.
  2. Open the form file in Adobe Reader.
  3. Fill in the form.
  4. Rename and save the form to your computer before uploading it to e.g. Diakle. This way teachers can see the field contents.

If a field doesn’t have enough space for your text, you should print the form and complete it by hand.

Registration for the academic year

Use this form only if you don’t have the possibility to register as attending or non-attending in MyDiak. Further information on registration.

The form is signed with authentication. For that, you need a mobile certificate or online banking IDs. If you do not have them, contact Diak’s Study Office (
